The contest entry form is available for download as a PDF file and as a .jpg file. To read the .pdf file, you will need Adobe® Acrobat® Reader™ installed on your machine. You may download the free software at the Adobe website.

Once you have downloaded and installed the software, you can download the contest entry form: • Download the entry form .pdf • OR • entry form.jpg

PRINTING THE ENTRY FORM
The entry form is designed to print on letter size paper in “landscape” (or “wide”) format.
Once you have printed the entry form, fill it out and attach it to your entry according to the instructions listed on this website. Checks should be made payable to MSU/SND.
